The present research aims to validate and culturally adapt the oral health impact profile instrument in Ecuadorian adults over 40 years of age; through a cross-sectional and longitudinal study using the OHIP 14Mx quality of life instrument as a reference. It was carried out in Quito, Ecuador analyzing sociocultural, clinical and self-perception variables. There was a sample of 150 people, 68 men (45.3%) and 82 women (54.7%); the mean age in men 59 years (± 9) and women 54 (± 9). Reliability by dimensions (median of 0.76 Cronbach's α), repeatability (0.99) and stability (0.99) were evaluated in the second pilot test in order to create the OHIP 14Ec. Self-perception of the need for treatment could have been affected since the entire sample was taken in a dental clinic. Periodontal disease (p = 0) and dental caries (p = 0.1) were the most frequent pathologies. Based on the results obtained from the OHIP14Ec, it is determined that it is a reliable and repeatable instrument for use in adults over 40 years of age. It meets the expectations to evaluate the quality of life of Ecuadorians.
