The human papillomavirus constitutes a heterogeneous viral group capable of producing hyperplastic, papillomatous and warty lesions in both skin and mucosa, and in recent years it has been shown to play an important role in carcinogenesis. During the last decades, human papillomavirus (HPV) infection has emerged as a new epidemic and has become a health problem due to its association with various cancers located in the cervix, anogenital region and oropharynx. The objective of this work is to report a clinical case of a patient of the sex, male of 65 years, smoker for more than 40 years, who appeared at the Department of Pathology of the San Gregorio University of Portoviejo, presenting an extensive white lesion with a marked growth warts on the edge, ventral face of the tongue and floor of the mouth on the right side, with no scraping. Considering the clinical characteristics of the lesion, the provisional diagnosis was verrucous leukoplakia versus verrucous carcinoma and histopathological evaluation confirmed the diagnostic hypothesis with histological picture compatible with Verrucous Leukoplakia (Keratosis with Moderate Intraepithelial Dysplasia).
