Esta condición amerita tratamiento de rescate y profiláctico. Muchos pacientes no responden al tratamiento profiláctico de primera elección. En Panamá desde 2015 se utiliza la toxina botulínica A en pacientes que no responden a la terapia profiláctica de primera elección. Objetivo : Determinar el impacto de la Toxina Botulínica A como terapia profiláctica en pacientes con migraña crónica en Panamá. Metodología : Estudio descriptivo retrospectivo. La muestra fue igual al universo: 29 participantes en protocolo PREEMPT 1. Los datos se recolectaron de expedientes clínicos. Se evaluaron características demográficas y clínicas, incluyendo las escalas HIT-6 y MIDAS. Resultados : La mediana de edad fue 43 años, todos los participantes pertenecían al sexo femenino. El 55.1% presentaban de 25 a 30 días con crisis agudas por mes. Utilizando la terapia, se identificó reducción de las crisis en un 89.66% y 93.10% a los 3 y 6 meses, respectivamente. No fueron reportados efectos adversos. Conclusiones : Los pacientes panameños con diagnóstico de migraña crónica resistente a la terapia profiláctica de primera elección, han respondido positivamente al tratamiento con toxina botulínica A, con una significativa mejoría en su calidad de vida.<hr/>Abstract Introduction: Chronic migraine limits the quality of life of patients. This condition requires rescue and prophylactic treatment. Many patients do not respond to first-line prophylactic treatment. Since 2015, botulinum toxin A has been used in Panama for patients who do not respond to first-line prophylactic therapy. Objective : Determine the impact of Botulinum Toxin A as a prophylactic therapy in patients with chronic migraine in Panama. Methodology: A retrospective, descriptive study. Sample was equal to universe: 29 participants in the PREEMPT 1 protocol. Data collection was based on reviewing medical records. The study evaluated demographic and clinical characteristics, including MIDAS and HIT-6 scales scores. Results : Participants had an average age of 43 years; all were female. A total of 55.1% experienced 25 to 30 days of acute migraine crises per month. Therapy shows a crisis reduction of 89.66% and 93.10% at 3 and 6 months, respectively. No adverse effects were reported. Conclusions : Panamanian patients with a diagnosis of chronic migraine resistant to first-line prophylactic therapy have responded positively to treatment with botulinum toxin A, with a significant improvement in their quality of life. <![CDATA[The Effectiveness Of Immersive Virtual Reality For Spatial Learning Tasks In Two Diverse Age Groups]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100024&lng=en&nrm=iso&tlng=en Abstract Spatial learning is a critical function for our effective functioning in the world. Despite the importance of this function, its study has been limited to unrealistic contexts, hindering a deep understanding of its underlying mechanisms. Today's technological advancements allow us to study this phenomenon in environments close to natural ones, with the potential to perform physiological measurements that help us understand its mechanisms in humans. In this work, we aimed to evaluate the box room task implemented in an Immersive Virtual Reality System (IVRS) for studying spatial learning in humans. The sample consisted of healthy children and older adults, without neurological or psychiatric alterations. All participants successfully completed the task without any problems associated with the use of IVRS. Although in different ways, both children and older adults showed improvements in the three established performance measures used to assess their performance over the course of the rounds: time, travel distance, and number of errors. Our results support the effectiveness of the box room task implemented in an IVRS for studying spatial learning and navigation in humans. The flexibility of the task positions it as a valuable tool for investigating spatial cognition in a variety of populations with diverse characteristics.<hr/>Resumen El aprendizaje espacial es una función crítica para nuestro funcionamiento efectivo en el mundo. A pesar de su importancia, su estudio ha estado limitado a contextos poco realistas, lo que dificulta una comprensión profunda de sus mecanismos subyacentes. Los avances tecnológicos actuales nos permiten estudiar este fenómeno en entornos similares a los naturales. En este trabajo, nuestro objetivo fue evaluar la tarea Sala de Cajas implementada en un Sistema de Realidad Virtual Inmersivo (SRVI) para estudiar el aprendizaje espacial en humanos. La muestra consistió en niños sanos y adultos mayores, sin alteraciones neurológicas o psiquiátricas. Todos los participantes completaron con éxito la tarea sin problemas asociados al uso del SRVI. Aunque de diferentes maneras, tanto los niños como los adultos mayores mostraron mejoras en las tres medidas de rendimiento establecidas para evaluar su desempeño a lo largo de las rondas: tiempo, distancia recorrida y número de errores. Nuestros resultados apoyan la efectividad de la tarea Sala de Cajas implementada en un SRVI para estudiar el aprendizaje espacial y la navegación en humanos. La flexibilidad de la tarea la posiciona como una herramienta valiosa para investigar la cognición espacial en una variedad de poblaciones con diversas características. <![CDATA[Neurophysiological And Neuropsychological Bases Implicated In Delinquent Behaviors: A Systematic Review]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100032&lng=en&nrm=iso&tlng=en RESUMEN Las estructuras neurofisiológicas y neuropsicológicas son factores que pueden estar implicados en el cometimiento de conductas delictivas, no obstante, su evaluación en el campo pericial forense está lejos de ser considerado una prueba o sustento en los casos judiciales en nuestro contexto. El objetivo de esta investigación fue identificar las bases neurofisiológicas y neuropsicológicas implicadas en las conductas delictivas a través de una revisión sistemática cuantitativa. Para ello, se tomó en cuenta investigaciones previas alojadas en Scopus y Pubmed. Por tal razón, se identificaron un total 15 artículos analizados a través de la aplicación de criterios de inclusión y exclusión. Los resultados sugieren que, la mayoría de estructuras neurofisiológicas implicadas en los delitos, provienen de zonas ligadas al lóbulo frontal y del sistema límbico o aledañas. De igual forma, se identificó que los individuos que cometieron los delitos presentan deficiencias neuropsicológicas relacionadas a la toma de decisiones, autorregulación, reconocimiento emocional, agresividad, anticipación, planificación, entre otras. En conclusión, se pudo identificar que efectivamente los evaluados presentan anomalías neurofisiológicas y neuropsicológicas, sin embargo, el cometimiento de las conductas delictivas depende también de otros factores biológicos y sociales. Finalmente, la revisión sistemática abre la brecha a la importancia de incluir la evaluación neuropsicológica forense para el esclarecimiento de los delitos, generando así una prueba más en los procesos judiciales.<hr/>ABSTRACT Neurophysiological and neuropsychological structures are factors that may be involved in the commission of criminal behavior, however, their evaluation in the forensic expert field is far from being considered as evidence or support in court cases in our context. The objective of this research was to identify the neurophysiological and neuropsychological bases involved in criminal behaviors through a quantitative systematic review. For this purpose, previous research housed in Scopus and Pubmed was taken into account. For this reason, a total of 15 articles were identified and analyzed through the application of inclusion and exclusion criteria. The results suggest that most of the neurophysiological structures involved in crimes come from areas linked to the frontal lobe and the limbic system or nearby. Likewise, it was identified that offenders present neuropsychological deficiencies related to decision-making, self-regulation, emotional recognition, aggressiveness, anticipation, planning, among others. In conclusion, it was possible to identify that those evaluated do indeed present neurophysiological and neuropsychological anomalies; however, the commission of criminal behaviors also depends on other biological and social factors. Finally, the systematic review opens the gap to the importance of including forensic neuropsychological evaluation for the clarification of crimes, thus generating one more piece of evidence in judicial processes. <![CDATA[Refractory Infantile Spasm Syndrome Treatment: A Systematic Review]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100047&lng=en&nrm=iso&tlng=en Abstract Introduction: Infantile spasm syndrome is an epileptic and neurodevelopmental encephalopathy whose first-line treatment includes vigabatrin and hormonal therapy. However, there are refractory cases. Objective: To compare the efficacy of the available treatments for refractory infantile spasm syndrome (RISS). Methods: We searched PubMed, Embase, Scopus, Web of Science, Cochrane CENTRAL Clinical Trials, International Clinical Trials Registry Platform (ICTRP) and ClinicalTrials.gov until March 2024 for randomized controlled trials (RCTs) comparing the efficacy of two of the following three treatment alternatives: antiepileptics, ketogenic diet and surgery. The primary outcome was electroclinical response. The GRADE methodology was used to appraise the certainty of the evidence. We generated a Summary of Findings table using the GRADEpro software. Results: We did not find enough RCTs to do a meta-analysis. Only one trial was close to fulfill our inclusion criteria. This study involved 318 patients with RISS receiving antiepileptic drugs and compared the modified Atkins diet (MAD) versus normal diet. However, we found a high risk of bias. Conclusion: High-quality RCTs comparing treatment modalities for patients with RISS are urgent due to the devastating nature of this condition.<hr/>Resumen Introducción: El síndrome de espasmos infantiles es una encefalopatía epiléptica y del neurodesarrollo cuyo tratamiento de primera línea incluye vigabatrina y terapia hormonal. Sin embargo, hay casos refractarios. Objetivo: Comparar la eficacia de los tratamientos para el síndrome de espasmos infantiles refractarios (SEIR). Metodología: Se realizaron búsquedas en PubMed, Embase, Scopus, Web of Science, Cochrane CENTRAL Clinical Trials, International Clinical Trials Registry Platform (ICTRP) y ClinicalTrials.gov hasta marzo de 2024 para encontrar ensayos controlados aleatorizados (ECAs) que compararan la eficacia de dos de las siguientes tres alternativas de tratamiento: antiepilépticos, dieta cetogénica y cirugía. El resultado primario fue la respuesta electroclínica. Se utilizó la metodología GRADE para valorar la certeza de la evidencia. Generamos una tabla Resumen de Hallazgos utilizando el software GRADEpro. Resultados: No se encontraron suficientes ECAs para realizar un meta-análisis. Sólo un ensayo estuvo cerca de cumplir los criterios de inclusión. En dicho estudio participaron 318 pacientes con SEIR que recibían fármacos antiepilépticos y se comparó la dieta Atkins modificada (DAM) con la dieta normal. Sin embargo, encontramos un alto riesgo de sesgo. Conclusión: Urgen ECAs de calidad que comparen las modalidades de tratamiento para pacientes con SEIR dada la naturaleza devastadora de esta afección. <![CDATA[Guillain-Barré Syndrome and its Pharyngocervicobrachial Variant: Unusual Presentation and Clinical Challenges"]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100054&lng=en&nrm=iso&tlng=en Resumen El síndrome de Guillain-Barré (SGB) abarca un amplio espectro clínico de signos y síntomas que pueden agruparse en variantes con distinta frecuencia de presentación. En este caso, destacamos la variante faringo-cervico-braquial (FCB) en un paciente sin antecedentes médicos relevantes que vive en una zona endémica de enfermedades transmitidas por vectores. Presentación del caso: Mujer de 57 años acude a urgencias con cuadro clínico de debilidad cervicobraquial y disfagia aguda de 4 días de evolución. Se realizó una punción lumbar que mostró una disociación albumino citológica, además se obtuvieron hallazgos neurofisiológicos compatibles con una polirradiculopatía desmielinizante aguda. Discusión: Esta variante fue descrita por primera vez en 1986 y es frecuentemente mal diagnosticada debido a su rareza y baja prevalencia. Una revisión de la literatura mostró que varios reportes de casos describieron la presencia de esta patología después de la infección por arbovirus. Conclusiones: Dentro del espectro del síndrome de Guillain-Barré, existe una variante descendente reportada en la literatura en pacientes con infecciones tropicales previas, lo que abre la puerta a futuras investigaciones para establecer si existe relación entre esta variante, las infecciones tropicales y su fisiopatología.<hr/>Abstract: Guillain-Barré syndrome (GBS) encompasses a variety of clinical manifestations, including the rare and easily underdiagnosed pharyngeal-cervical-brachial variant (PCB). Here, we present the case of a 57-year-old woman with no significant medical history who lives in an area endemic for vector-borne diseases. She presented to the emergency department with four days of cervicobrachial weakness and acute dysphagia. Although imaging was normal, lumbar puncture revealed albuminocytologic dissociation and neurophysiological tests demonstrated acute demyelinating polyradiculopathy, confirming the diagnosis of the CPB variant. First described in 1986, this variant has been reported following arbovirus infections, such as dengue and chikungunya. This case highlights the importance of considering this atypical form of GBS and suggests the need for future research on its possible association with vector-borne infection. <![CDATA[New Possible Mutations For Aceruloplasminemia]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100058&lng=en&nrm=iso&tlng=en RESUMEN La aceruloplasminemia es una entidad ultrarrara y de inicio tardío dentro del grupo de enfermedades neurodegenerativas con acumulación cerebral de hierro (NBIA). There are new mutations to describe, such as the one in this clinical case. <![CDATA[Atypical Presentation Of Creutzfeldt-Jakob Disease. A Case Report In Chilean Patagonia.]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100060&lng=en&nrm=iso&tlng=en Resumen: Introducción: La enfermedad de Creutzfeldt-Jakob (ECJ) es una enfermedad neurodegenerativa fatal. Sus síntomas incluyen demencia de rápida progresión, mioclonías y alteraciones motoras. En Chile, se observa una incidencia notablemente alta, lo cual, junto con su letalidad, resalta la importancia del diagnóstico precoz. Reporte de caso: Mujer de 64 años, que consultó por vértigo, inestabilidad de la marcha, disartria y diplopía. El examen físico reveló ataxia, dismetría y disdiadococinesia. Se solicitaron resonancia magnética y electroencefalograma, cuyos hallazgos fueron sugestivos de ECJ. La condición de la paciente se deterioró rápidamente, desarrollando demencia a los 3 meses y falleciendo al cuarto mes. Discusión y conclusión: Las variantes de ECJ pueden presentarse sin alteración de funciones cognitivas y con pródromos inespecíficos. El diagnóstico requiere un alto índice de sospecha, considerando como pruebas esenciales la resonancia magnética nuclear y la medición de la proteína 14-3-3 en el líquido cefalorraquídeo. El aumento de incidencia en Chile podría deberse a mutaciones y factores ambientales, sugiriendo la necesidad de investigar a fondo las causas y desarrollar tratamientos adecuados.<hr/>Abstract Introduction: Creutzfeldt-Jakob disease (CJD) is a fatal neurodegenerative disease. Symptoms include rapidly progressive dementia, myoclonus and motor impairment. In Chile, there is a remarkably high incidence, which together with the lethality of it highlights the importance of early diagnosis. Case report: A 64-year-old woman presented with vertigo, gait instability, dysarthria and diplopia. Physical examination revealed ataxia, dysmetria and dysdiadochokinesia. Magnetic resonance imaging and electroencephalogram were ordered, with findings suggestive of CJD. Patient's condition deteriorated rapidly, developing dementia after 3 months and dying in the fourth month. Discussion and conclusion: CJD variants may present without alteration of cognitive functions and nonspecific prodromes. Diagnosis requires a high index of suspicion, taking as essential tests the nuclear magnetic resonance and the measurement of 14-3-3 protein in cerebrospinal fluid. The increased incidence in Chile may be due to mutations and environmental factors, suggesting the need to thoroughly investigate the causes and develop appropriate treatments. <![CDATA[Hashimoto´s Encephalitis]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100065&lng=en&nrm=iso&tlng=en Resumen Introducción: La encefalitis de Hashimoto (EH) es una entidad clínica que se caracteriza por la presencia de alteraciones neurológicas asociado a la presencia de anticuerpos antitiroideos positivos. Esta encefalopatía responde bien al manejo con corticoides. Objetivo: El objetivo del manuscrito es presentar el caso de un paciente con encefalitis de Hashimoto. Caso clínico: Se trata de una mujer de 18 años sin antecedentes de importancia, quien ingresó por cuadro clínico de 5 días de evolución que inició con fotopsias, episodios convulsivos recurrentes tónico-clónicos generalizados, con recuperación de la conciencia intercrisis. Al ingreso estaba taquicárdica, desorientada, sin respuesta durante el interrogatorio, pupilas simétricas, con presencia de nistagmo vertical, mirada preferencial hacia la derecha, movimientos clónicos faciales y de miembro superior izquierdo. Los paraclínicos y estudios imagenológicos de ingreso estaban normales. Al tercer día de ingreso la paciente presentó estado epiléptico con falla ventilatoria por lo que se decidió manejar coma barbitúrico. La punción lumbar mostraba hiperproteinorraquia como único hallazgo, los estudios de extensión descartaban una encefalitis autoinmune, sin embargo, presentaba hipotiroidismo subclínico con anticuerpos antiperoxidasa (Anti-TPO) positivos. Ante esto se sospechó que cursaba con encefalitis de Hashimoto, por lo que se dio manejo con corticoides (prednisolona 50 mg cada día) con lo que se obtuvo una adecuada evolución clínica de la paciente. Conclusiones: Esta es una enfermedad de baja prevalencia, con síntomas neurológicos y psiquiátricos variados, donde uno de los criterios de mayor importancia que tiene esta entidad patológica es la presencia de anticuerpos antitiroideos positivos con buena respuesta al manejo con corticoides.<hr/>Abstract Introduction: Hashimoto's encephalitis (HD) is a clinical entity characterized by neurological alterations associated with the presence of positive antithyroid antibodies. This encephalopathy responds well to management with corticosteroids. Objective: The objective of the manuscript is to present the case of a patient with Hashimoto's encephalitis. Clinical case: This is an 18-year-old woman with no significant history, who was admitted for a 5-day history that began with photopsias, recurrent generalized tonic-clonic seizure episodes, with recovery of consciousness between seizures. On admission she was tachycardic, disoriented, unresponsive during questioning, symmetrical pupils, with presence of vertical nystagmus, preferential gaze to the right, clonic facial and left upper limb movements. Paraclinical and imaging studies on admission were normal. On the third day of admission, the patient presented status epilepticus with ventilatory failure, so she decided to manage a barbiturate coma. Lumbar puncture showed hyperproteinorrachia as the only finding; extension studies ruled out autoimmune encephalitis; however, she had subclinical hypothyroidism with positive antiperoxidase antibodies (Anti-TPO). Given this, it was suspected that she had Hashimoto's encephalitis, so treatment with corticosteroids (prednisolone 50 mg every day) was given, resulting in an adequate clinical evolution of the patient. Conclusions: This is a low prevalence disease, with varied neurological and psychiatric symptoms, where one of the most important criteria for this pathological entity is the presence of positive antithyroid antibodies with a good response to management with corticosteroids. <![CDATA[Lafora Disease: First Genetically Confirmed Case In Ecuador]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100068&lng=en&nrm=iso&tlng=en Resumen La enfermedad de Lafora es una epilepsia mioclónica progresiva autosómica recesiva, caracterizada por crisis tónico-clónicas generalizadas refractarias, mioclonías persistentes, deterioro cognitivo grave y progresivo, y evolución fatal. Presentamos el primer caso confirmado genéticamente en Ecuador. Se trata de un paciente masculino de 17 años con crisis tónico-clónicas generalizadas desde los 13 años, refractarias al tratamiento farmacológico combinado. Presenta deterioro cognitivo grave, regresión neuropsicológica importante y un coeficiente intelectual de 32. El electroencefalograma (EEG) mostró actividad epileptiforme generalizada compatible con epilepsia mioclónica progresiva. Se confirmó una mutación en el gen NHLRC1 y se indicó dieta cetogénica como complemento terapéutico. Este reporte enfatiza la importancia del diagnóstico genético temprano y del abordaje multidisciplinario integral.<hr/>Abstract Lafora disease is an autosomal recessive progressive myoclonic epilepsy characterized by refractory generalized tonic-clonic seizures, persistent myoclonus, severe and progressive cognitive decline, and a fatal course. We present the first genetically confirmed case in Ecuador. A 17-year-old male developed generalized tonic-clonic seizures at age 13, refractory to combined pharmacological treatment. He exhibited severe and progressive cognitive decline, significant neuropsychological regression, and an IQ of 32. The electroencephalogram (EEG) showed generalized epileptiform activity compatible with progressive myoclonic epilepsy. Genetic testing confirmed a mutation in the NHLRC1 gene. A ketogenic diet was initiated as a complementary therapeutic approach. This case highlights the importance of early genetic diagnosis and a comprehensive multidisciplinary management strategy. <![CDATA[Cryptococcosis with immune-mediated central nervous system vasculitis: A diagnostic challenge]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100071&lng=en&nrm=iso&tlng=en Resumen: Se presenta el caso de un hombre de 63 años que consultó inicialmente por hipoacusia y neuritis óptica. Posteriormente desarrolló vértigo y se documentaron múltiples infartos cerebrales multitopográficos, con estudios de factores de riesgo vasculares negativos. La panangiografía mostró hallazgos sugestivos de vasculitis del sistema nervioso central y la punción lumbar inicial no evidenció infección, por lo que se consideró una vasculitis mediada inmunológicamente. Tras un nuevo deterioro neurológico, se confirmó infección del sistema nervioso central por Cryptococcus neoformans. Este caso resalta la dificultad diagnóstica y las limitaciones de las pruebas microbiológicas iniciales en criptococosis del SNC.<hr/>Abstract: We report the case of a 63-year-old man who initially presented with hearing loss and optic neuritis. He subsequently developed vertigo, and multiple multitopographic cerebral infarctions were documented, with negative vascular risk factor evaluation. Panangiography revealed findings suggestive of central nervous system vasculitis, and the initial lumbar puncture showed no evidence of infection; therefore, an immune-mediated vasculitis was considered. Following further neurological deterioration, central nervous system infection due to Cryptococcus neoformans was confirmed. This case highlights the diagnostic challenge and the limitations of initial microbiological testing in central nervous system cryptococcosis. <![CDATA[Basal Ganglia Calcifications Consistent With Fahr Syndrome In An Autoimmune Context]]> http://scielo.senescyt.gob.ec/scielo.php?script=sci_arttext&pid=S2631-25812026000100076&lng=en&nrm=iso&tlng=en Resumen: Se presenta el caso de un hombre de 63 años que consultó inicialmente por hipoacusia y neuritis óptica. Posteriormente desarrolló vértigo y se documentaron múltiples infartos cerebrales multitopográficos, con estudios de factores de riesgo vasculares negativos. La panangiografía mostró hallazgos sugestivos de vasculitis del sistema nervioso central y la punción lumbar inicial no evidenció infección, por lo que se consideró una vasculitis mediada inmunológicamente. Tras un nuevo deterioro neurológico, se confirmó infección del sistema nervioso central por Cryptococcus neoformans. Este caso resalta la dificultad diagnóstica y las limitaciones de las pruebas microbiológicas iniciales en criptococosis del SNC.<hr/>Abstract: We report the case of a 63-year-old man who initially presented with hearing loss and optic neuritis. He subsequently developed vertigo, and multiple multitopographic cerebral infarctions were documented, with negative vascular risk factor evaluation. Panangiography revealed findings suggestive of central nervous system vasculitis, and the initial lumbar puncture showed no evidence of infection; therefore, an immune-mediated vasculitis was considered. Following further neurological deterioration, central nervous system infection due to Cryptococcus neoformans was confirmed. This case highlights the diagnostic challenge and the limitations of initial microbiological testing in central nervous system cryptococcosis.
